「Education Sessions：乳癌ネオアジュバント療法の集学的治療―今日の課題」の座長Monica Morrow氏に聞く
会期の最終日だったにも関わらず，広い会場を埋め尽くす盛況となった本教育セッションでは，乳癌の手術前に腫瘍縮小を目的に行われる化学療法（ネオアジュバント療法）について討議された。座長のMonica Morrow氏（メモリアル・スローン・ケタリング癌センター）は，自らも「ネオアジュバント療法を受ける患者へのセンチネルリンパ節生検はいつ行うべきか：化学療法の前か後か？」と題して講演。テーマとなったセンチネルリンパ節とは癌細胞が最初に流れつくリンパ節のことで，リンパ節転移の有無は，これを調べることで知ることができる。つまり，生検結果が陰性であれば，無駄な腋窩リンパ節郭清を行わなくて済む可能性がある。わが国でも多くの施設が採用するようになったが，保険収載は来年と見込まれている。一方，米国ではFDAが2007年に承認しており，その実施経験は長い。そこでMorrow氏に，米国でのセンチネルリンパ節生検の現状や実施時の注意点を聞いた。（越山みどり）

axillary dissection：腋窩（リンパ節）郭清
So, we were discussing the timing of sentinel node biopsy in neoadjuvant chemotherapy, and I think at this point in time for the clinically node-negative patient at presentation sentinel node biopsy after neoadjuvant therapy provides accurate staging and local control and is a safe thing to do.
For the patient who presents with metastatic disease in the nodes which is proven by manual aspiration, even if they appear to downstage with chemotherapy, axillary dissection remains the standard of treatment.

Yes. Yes. We use sentinel node biopsies the routine way for staging the axilla in patients, certainly, who are having surgery without chemotherapy, but at this point in time I think most people are probably doing it after chemotherapy as well.

Well, the practical importance is that by doing the sentinel node biopsy after chemotherapy you will save approximately 30% of women from an axillary dissection with its side effects. From a technical point of view, you need to be careful when you are doing it to make sure that you feel in the axilla to make sure there are no abnormal nodes that don't take up the radioactive tracer or the blue dye, because if you don't do that you may miss bulky nodal disease.
